The derating Graph is for a 2N2222A [2N2221A] in a surface mount package. Use the links below for additional derating information covering different package styles. The most common surface mount package would be a SOT23 or SOT-223 device but is not qualified by this military specification. So the package device defined here is a very different package style, as shown below.
The 2N2222 in this package style will operate up to 50C with out the need for derating.
PWB layout hint; increase the pad size to dissipate more heat into the board and away from the 2N2222 package.

The UA package is a 4 tab surface mount package. However only one side [two pins] are shown in this view.
Editor Note: I can't tell how common this particular surface mount package is. The package is defined in government or Department of Defense documents as an approved package for both this transistor and others [defined on other pages]. However I have never come across this particular package, either in a design function or some other debug of some one else's card. Common 2N2222 surface mount package styles are; PN2222, or MMBT2222A, and PZT2222A.
Of course they have added this package style to the DOD specification for a reason. And if the design happens to be required to comply with military specifications, than this SMD component would be required. As any plastic 2N2222 surface mount package would not be allowed. So the cost difference between the cheaper plastic version and the more expensive ceramic military version would not matter. As the ceramic version would be required by design, or by requirement.
