A Standoff would be considered to be much taller than a spacer or washer.
However there is no hard and fast rule of what height constitutes a Standoff.
A standoff is used to separate two surfaces, which could occur with a spacer.

A Standoff is a part of various configurations which facilitates the assemblage and/or fastening of components. It is designed to maintain a predetermined distance between component parts and/or provide easy access into equipment or to align and extend the length of threaded items. A standoff is normally made of metal or plastic, although different materials are available. A standoff may be threaded at one or both ends, or internally threaded its full length.
A threaded snap-in stand-off is a standoff with one threaded end and projection on the other end which can be snapped, press fitted or locked into position.
